ASSESSMENT AND PLAN:
The patient is a
54-year-old female with a history of acute low back pain. After the ayurvedic
examination, this client was diagnosed with suffering from Gridhrasi
(sciatica), and a composite Ayurvedic treatment was given. Patrapinda Swedana
for seven days and Kati Basti for seven days schedule were used as Panchakarma
procedure
Step 1-
Ruksha Pottali Sweda
(dry bolus fomentation in katipradesh (low back region) and full body
Step 2-
Further Kati-Basti (a
procedure where specified medicated oil is
retained for a
stipulated period in the Kati region) with herbal oil.
RESULTS
Walking distance:
- Before treatment: - Patient had severe pain after
walking 100 mts
- After treatment: - Patient could easily walk without pain
about 200 mts
- Sitting on the sofa
- the treatment patient was not able to sit on the
sofa

Discussion
According to Ayurveda,
Gridhrasi is a caused due to imbalance of Vata dosha, which results in
intervening with the functional ability of low back & lower limbs.
According to Arundutta, in his commentary, it is mentioned that due to Vata
dosha in Kandara (tendon) the pain is produced at the time of raising leg
straight and it restricts the movement of the thigh
In modern science,
there is a condition called sciatica. The sciatica is the distribution of pain
along the course of the sciatic nerve or its component nerve roots. Radiating
deep-seated cramping pain in buttocks followed with numbness and paresthesia in
lower extremities favors the diagnosis. Restricted SLR test consolidates the
diagnosis clinically, and imaging techniques can confirm even the illness.
Patra Pinda Sweda
(bolus fomentation) is used mainly to provide relief from pain, inflammation,
swelling, and stiffness (catch) associated with bone, joint, and or
musculoskeletal pains. (8)
In Kati Basti (local
oil retention ), sukhoshna taila (lukewarm oil) is used over the Kati Pradesh.
Sneha (oil) by its Snigdha (smoothness), guru (heaviness) and ushna Guna
(hotness)
counteracts Vata and
by using guna prevents Kapha also.(9).

Based on this single
case study it can be concluded that Panchakarma treatments like Kati Basti and
full-body polite massage had been useful in the management of
Gridhrasi(sciatica)
