A male patient 35 years came in the shalya OPD of GAM&RC Shiroda GOA with bleeding during defecation and mucous discharge, semisolid and soft intact stools alternate days since 5-6 months. He also gave history of sour retching and regurgitation and occasionally pain in abdomen after food. His occupation is electrician and has to do night shifts hence has to eat his food late at night around 10-11pm and night disturbed sleep.
Diet fish 3-4 times a week and lot of vidahi (sour-salty-pungent) food. On examination vitals were within normal limits, clinically not anemic, tongue-coated(saam) prakruti- vadapradhan kaphanubandhi abdomen- non tender, L/S/K- non tender not palpable. Bowel sounds normal.
PR- proctoscopy- anal mucosa normal, no abnormality seen colonoscopy.
DIFFUSE ULCERATIVE COLITIS OF RECTUM, REST OF THE COLON IS NORMAL. HISTOPATHOLOGY-NO E/O TB AND MALIGNANCY. Nidaan(diagnosis)- kapha-pittaja grahani.
Chikitsa- paachan-raktasthambana(hemostasis) date- 13th October 2015
Internally-1) Musta+nagkeshar+bilwa +Rasa Parpati 200 mg each two times a day, samaan kaal(in between food) in lunch and dinner with go ghrita.
2) Praval Panchamruta vati 250mg 1-1-1 after food
3) Anuvasan basti (60ml)- jatyadi taila(30ml) + mahatiktak Ghrita(30ml).
During basti patient was advised to change his shift in the day to evening, avoid sexual intercourse, night working, exercise, cold water bath and drinking cold water.
Diet advised- cooked rice with moong gravy, raw red soup/vegetable, buttermilk freshly prepared and not too sour.
Diet contraindicated- fruits at night, cold water, non veg food, starch food.
Follow up- 14/10/15- patient passed semi solid stool with mild bleeding and mucous, he took basti in the afternoon at 12pm and basti pratyagatma (exit) took place in the evening 6pm with little oil n mucous.
15/10/15- patient passed stool in the morning at 7am, it was intact and there was no mucus and blood with stool, burning sensation in the abdomen got reduced, no bloating and acid regurgitation.
